PS. we are using shared user id to log in to the sql server. So only way is to find out IP address or something. The best you will get is when it was created. In SSMS, right-click the SQL Server instance, select Reports > Standard Reports > Schema Changes Report.

When the login u007 is granted access to a database in SQL Server, it creates a database user which maps back to the instance level login.
